Specimen Required

Patient Preparation: 

Specimen Type: Rectal, perirectal

Collection Container/Type: Culture transport swab (dual swab, LQ Stuart, plastic applicators, rayon tipped; Cepheid catalog number 900-0370).

Specimen Volume: Swab

Collection Instructions: 

  1. Rectal collection: Carefully insert both swab tips approximately 1 cm beyond the anal sphincter and rotate gently
     - OR -
    Perirectal collection: Carefully inser both swab tips no more than 1 cm into the anal opening before the anal sphincter and rotate gently.
  2. Heavily soiled swabs are unaccpetable.  See acceptable swabs below.
  3. Label swab with patient information.
  4. Transport o the laboratory at room temperature.

Clinical Significance

This test utilizes real-time PCR for the detection and differentiation of gene sequences associated with carbapenem-non-susceptibility in Enterobacteriaceae, Pseudomonas aeruginosa and Acinetobacter baumannii. Targets include blaKPC, blaNDM, blaVIM, blaOXA-48 (and blaOXA-181), and blaIMP (excluding IMP-7, IMP-13, IMP-14). Detection of these gene sequences does not indicate the presence of viable organisms. This assay does not report variants of these gene sequences. Mutations or polymorphisms may affect detection of current, new, or unknown variants, resulting in a false negative result. This test is for Infection control surveillance purposes only and should not be used to make treatment decisions. The performance of the Xpert Carba-R Assay has not been evaluated with rectal swab specimens from pediatric patients.

Testing Algorithm

This assay allows for the detection and differentiation of the blaKPC, blaNDM, blaVIM, blaOXA-48, and blaIMP gene sequences associated with carbapenem-non-susceptibility. If gene sequences are detected, culture will automatically performed on the remaining specimen to attempt to identify the organism for infection control purposes.
